The relational power of home visiting, combined with culturally responsive and targeted interventions in an antiracist system of care, can help to advance health equity across the Maternal, Infant, and Early Childhood Home Visiting (MIECHV) Program. The Health Equity CoIIN supports MIECHV awardees and local implementing agencies to build their capacity to advance and sustain health equity with families served by home visiting.
Health equity demands that MIECHV programs remove obstacles to health—such as poverty, discrimination, and the consequences of both—with the goal of eliminating inequities in the key family outcomes that home visiting aims to improve.
The playbook outlines the Key Driver Diagram, Change Package, and Measurement System used by the Health Equity CoIIN.
Advancing health equity is an ongoing process. As the HV CoIIN team, awardees, and local home visiting agencies continue to test and refine the Health Equity Framework, preliminary resources are being shared. This working draft of the HV CoIIN Health Equity Toolkit is designed to support state, territory, tribal, and local teams, and individuals in examining and acting toward advancing health equity in MIECHV.
